Spoilers Below!Plot:
The story begins in Megasaki City, where Mayor Kobayashi banishes all dogs to Trash Island after an outbreak of an influeza virus spreads throughout the canine population, that could possibly spread to humans. The first dog to be exiled to Trash Island is Spots, which was owned by Atari, the orphaned nephew and ward of the Mayor. Then, the point of view is switched to Chief, who's the leader of a pack of dogs. He was formerly a stray before being exiled to Trash Island, unlike the other dogs of the pack, who were pets and are unfamiliar to scrounging for scraps of food.

One day, they see a plane crash and find Atari, who's on a mission to find Spots. The pack decides to assist Atari on his mission and they set off looking for any clues that might lead him to Spots. Chief is wary of Atari as he's very hesitant to engage with humans. They fend off a rescue team sent by Atari's uncle, Kobayashi to bring him back. Chief has doubts whether he should go with Atari and the rest of the pack to look for Spots, but is encouraged to do it by another dog called Nutmeg. They journey to two wise dogs, Jupiter and Oracle, who warn them of an isolated tribe of cannibal dogs. Subsequently, we see that Professor Watanabe has found a cure to dog flu and presents his discovery to the Mayor, only for him to refuse to lift the dog ban. He's put on house arrest and is killed by poison in his food. The story switches to Tracy Walker, an American exchange student who doubts that the Mayor is really telling the truth about the dog flu and Atari's situation.

During the trip to find the tribe of cannibal dogs, Atari and Chief find themselves separated from the rest of pack. They begin to get closer and Atari gives Chief a bath, they learn that Chief's coat is actually white. Due to their similarity, Atari realizes that Chief must be related to Spots. They finally join up with the rest of their group, but are ambushed by Kobayashi's men. The tribe of cannibal dogs come to their rescue. The movie shows us a flashback of what happened to Spots after being exiled to Trash Island. It turns out he was rescued by the tribe of "cannibal" dogs and is now the leader and expecting children with another dog, Peppermint. Spot confirms the question we've been wanting to ask, he is in fact Chief's brother and he asks Chief to take over his protection duties for Atari, which he agrees to.

A black owl comes to the group and tells them the message given him to by Jupiter. The Owl tells them that Mayor Kobayashi plans to exterminate all the dogs on Trash Island if reelected. Atari and the dogs plan to go to Megasaki City for election night. Tracy confronts Professor Watanabe's former assistant, Yoko Ono, who confirms her doubts and she hands over the last remaining vial of the cure to her. At the re-election ceremony, Tracy delivers her evidence to Mayor Kobayashi, who promptly denies everything and orders for her deportation to the United States of America. The Mayor's right hand man, Major Domo, hands over the red button that'll trigger the extermination on Trash Island. However, Atari and the dogs interrupts the ceremony causing it to come to a stand still. He makes a speech to the crowd and finishes it off with a heartbreaking haiku. Mayor Kobayashi revokes the decree and Major Domo is enraged by his decision and a fight ensues. Major Domo presses the red button only to learn that in fact, the poison backfires on the men due to help from Tracy's hacker friend.

Ultimately, both Atari and Spots are critically injured in the fight. Atari's remaining kidney fails and Mayor Kobayashi decides to give him a kidney to save his life. The control of the city falls onto Atari as he's the only heir suitable to undertake this role. However, his uncle, Major Domo and political allies are imprisoned for their corruption. Tracy and Atari start to date and Chief reunites with Nutmeg, in his new role as Atari's bodyguard. Spots, who has been presumed dead, recovers from his injuries to raise his litter with Peppermint under the Kobayashi Manor.
